Position Overview:

Provides direct and indirect patient care to individuals with neurodegenerative diseases and their families, utilizing the nursing processes of comprehensive patient assessment, development and implementation of nursing care plans, patient and caregiver education, and on-going evaluation. Delivers holistic, individualized care that supports both the patient and their caregiver network, recognizing the progressive, complex nature of neurodegenerative conditions.

Job Description:

Key Responsibilities

Patient Care & Assessment

  • Conduct thorough assessments that incorporate cognitive, functional, neurological, psychosocial, and safety considerations unique to neurodegenerative patients.
  • Identify care needs that may evolve as the disease progresses (e.g., mobility, swallowing, behavioral changes, caregiver stress).
  • Provide direct clinical support during patient visits, including infusion monitoring, medication administration, and coordination of diagnostic testing (MRI, PET, lab work).

Care Planning & Implementation

  • Develop, implement, and coordinate nursing care plans tailored to patients with cognitive decline, memory loss, Mild Cognitive Impairment, Alzheimer’s disease, Dementia(s), and other related disorders.
  • Integrate psychological, sociocultural, spiritual, economic, and lifestyle factors into care planning.
  • Educate patients and families on disease management, safety strategies, medication regimens, infusion therapy, and available community resources.

Collaboration & Communication

  • Act as a core member of the multidisciplinary care team, collaborating with neurologists, neuropsychologists, social workers, Gero psychologists, PT/OT, dietitians, and Alzheimer’s Association liaisons.
  • Foster strong partnerships with caregivers, recognizing their role as essential members of the care team.
  • Ensure effective, timely communication between patients, families, and the care team regarding disease status, treatment options, and care transitions.

Quality & Professional Practice

  • Adhere to clinic policies, nursing standards, best practices, and applicable regulations to promote a safe, reliable, and high-quality patient experience.
  • Document accurately and consistently in the EMR, including patient education, caregiver input, and observed changes in cognitive or functional status.
  • Engage in continuous growth and development in professional nursing practice, with an emphasis on advances in neurodegenerative care.

Qualifications

  • Current RN license in good standing.
  • BSN preferred; equivalent experience considered.
  • BLS-Basic Life Support Healthcare Provider
  • Experience in neurology, geriatrics, memory disorders, palliative care, or infusion nursing strongly preferred.
  • Strong clinical assessment skills and ability to respond to changing patient conditions.
  • Excellent interpersonal and communication skills, with sensitivity to the needs of cognitively impaired patients and their caregivers.
  • Proficiency in EMR systems; Epic experience preferred.

TYPICAL EXPERIENCE:

  • 2 years recent relevant experience

Pay Range is $78.62 to $103.77 / hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
