The role of the Clinical Referral Specialist is to act as the in-office clinical liaison that represents the SCAH agency via telephone receiving and processing referrals for patient care in the regional home health referral management team. Referral source interactions include, but are not limited to working closely with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families, and the general public. Primary clinical responsibilities include receiving/processing referrals and ensuring that admissions occur in a timely and consistent manner. The Clinical Referral Specialist enters referral information into the EMR and creates initial orders to facilitate the start of care. As the Clinical Referral Specialist, this person frequently interacts with referral sources, other members of the referral center staff, home health, AIM and hospice staff across SCAH offices and all others necessary to facilitate the start of patient care. Pay Range is $48.91 to $65.72 / hourThe compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidateβs experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
