Purpose:
The Nurse Case Manager coordinates and supports the rehabilitation of injured workers in a timely, cost-effective way. This role uses clinical expertise and independent judgment to assess cases, build care plans, coordinate services, and influence return-to-work outcomes while advocating for the injured workerβs physical and emotional well-being.
Key Responsibilities:
- Assess medical status and develop treatment plans with providers.
- Coordinate care across internal teams and external providers to ensure compliance and cost control.
- Recommend interventions and adjust plans as conditions change.
- Communicate with injured workers, providers, employers, and insurance representatives.
- Attend or coordinate medical appointments as needed.
- Work with employers on job duty modifications based on restrictions and functional abilities.
- Explain medical instructions and educate injured workers to support recovery and return to work.
- Serve as liaison and advocate throughout the case.
- Build and maintain referral relationships and represent VONA at meetings or conferences as needed.
- Provide insights to leadership on complex cases and process improvements.
- Maintain licensure and stay current on best practices and workersβ compensation requirements.
- Complete monthly progress reports and required documentation, including time tracking.
Requirements
- Active RN license.
- Case management experience required; workersβ compensation or occupational health preferred.
- CCM certification preferred.
- Strong clinical knowledge, medical terminology, and decision-making skills.
- Experience in hospital or rehabilitation settings preferred.
- Knowledge of HIPAA and confidentiality standards.
- Bilingual or multilingual skills are a plus.
