Role Summary: As an Account Manager / Sales Representative, you will serve as the primary liaison between OPO Health and our community partners. You'll represent our services to hospitals, physician practices, skilled nursing facilities, assisted living communities, board and care homes, and other healthcare professionals. You will play a key role in increasing referrals, driving admissions, and supporting patient continuity of care. 


Key Responsibilities: 

  • Establish and maintain strong relationships with referral sources and community partners.
  • Identify new referral opportunities and nurture them into consistent partnerships.
  • Educate physicians, discharge planners, case managers, and facility staff on OPO Health services and care coordination processes.
  • Meet or exceed monthly sales targets.
  • Conduct community outreach, attend networking events, and represent OPO Health at conferences and partner meetings.
  • Collaborate with internal clinical and intake teams to ensure timely, high-quality patient onboarding.
  • Track performance metrics and report regularly to leadership.
  • Maintain accurate documentation of referral activities using CRM tools.
Requirements
  • Minimum 1 year of proven sales experience in a home health setting (required)
  • Consistent track record of achieving or exceeding referral and admission targets 
  • Excellent interpersonal, communication, and presentation skills
  • Strong organizational and time-management abilities
  • Self-starter with a growth mindset and team-oriented attitude
  • Valid Driver’s License, Auto Insurance, and reliable transportation (required)
