Position Summary:

We are seeking a compassionate and reliable Licensed Vocational Nurse (LVN) to join our team. The LVN will provide direct nursing care and support to patients under the supervision of a physician, ensuring high-quality, patient-centered care that aligns with our organization’s standards and regulatory requirements.


Essential Duties and Responsibilities: 

  • Provide basic nursing care and administer medications and treatments as prescribed.
  • Process medication refills in accordance with established protocols and under the direction of the provider.
  • Monitor and document patients’ vital signs, symptoms, and responses to treatment.
  • Assist with patient assessments and report changes in condition to the provider team promptly.
  • Maintain accurate and timely medical records in accordance with facility policies and state/federal regulations.
  • Support patient education and promote health and wellness within the scope of licensure.
  • Coordinate with multidisciplinary team members to ensure continuity of care.
  • Maintain a safe and clean working environment; adhere to infection control and safety protocols.
  • Participate in staff meetings and quality improvement initiatives as required.


Qualifications:

  • Current and valid LVN license in the state of California 
  • CPR/BLS certification 
  • Experience in a clinical, long-term care, or outpatient setting preferred
  • Strong communication, documentation, and organizational skills.
  • Dependable and dedicated to providing quality patient care.


Language skills: 

  • Must possess excellent organization, writing, and verbal skills
  • English proficiency required


Reasoning Ability:

  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

 

Equipment/Machinery: Fax machine, copier, computer, telephone, EHR system, sterilization equipment and other software and instruments as required.

  1. Stand and walk or sit alternatively, depending on the specific needs of the day. Estimate 50% of time is spent on feet and 50% sitting at the desk and/or chairside.
  2. Have occasional need to perform the following physical activities: bend/stoop/squat, climb stairs, push or pull, reach above shoulders.
  3. Have an occasional need to perform standing and walking activities.
  4. Constant need to perform the following physical activities: writing/typing, grasping/turning, finger dexterity.
  5. Lifting/carrying over 20 pounds occasionally. Lifting/carrying less than 10 pounds frequently.
  6. Vision requirements: constant need to complete forms, read reports, and view the computer screen.  Frequent need to see small details.  Frequent need to see things clearly beyond arm's reach.
  7. Hearing requirements: constant need to communicate over the telephone and in person.
