Position Overview
We are seeking a dedicated Nurse Case Manager to support a coordinated and patient-centered healthcare delivery system. This role involves working collaboratively with multidisciplinary teams to assess, plan, coordinate, and evaluate patient care across the healthcare continuum.
You will serve as an advanced clinical resource for patients, families, physicians, and healthcare staff to ensure high-quality care and optimal outcomes

Requirements
Education & Experience:
- Graduate of an accredited nursing program
- Minimum 3 years of nursing experience in an acute care hospital
- At least 1 year in Case Management / Discharge Planning / Utilization Review
- Master’s Degree in Nursing (Case Management focus) may substitute for experience (optional)
License & Certification:
- Active Registered Nurse (RN) License – Nevada
- BLS Certification (American Heart Association)
Preferred Qualifications (Plus Points)
- 3+ years of case management experience
- Acute care experience (recent)
- Pediatric case management experience
- Emergency Department experience
- Utilization Review experience
- Certifications like:
- CCM (Certified Case Manager)
- ACM (Accredited Case Manager)
Key Skills & Knowledge
- Strong understanding of case management principles
- Patient care planning & evaluation
- Knowledge of healthcare regulations & reimbursement systems
- Ability to analyze medical records
- Excellent communication & teamwork skills
- Proficiency in computer systems and healthcare software
Work Environment & Physical Requirements
