Program Summary:
The Wellness Clinic is a core function of San Francisco Community Health Center. In 2015, we became the first new Federally Qualified Health Center (FQHC) in San Francisco in 40 years, enabling us to expand our capacity and provide full primary care services. The Wellness Clinic works in close collaboration with the Street Medicine team, Trans Thrive, Taimon Booton Navigation Center, and our other sites to coordinate primary care for patients accessing SFCHC services in multiple settings
Role Summary:
The Clinic Nurse (RN) will plan, coordinate, and oversee our day-to-day nursing services. They will support our primary care providers in patient adherence to treatment goals through coordination of care. In addition to clinical support, the RN will help us achieve our administrative goals. Opportunities will be available for professional growth and development through participation in educational programs, review of current literature, in-service meetings, and workshops.
Essential Duties:
- Performs timely client/patient telephone advice and triage assessment with clinical competence and judgment for urgent/drop-in care.
- Documents appropriately in electronic medical record.
- Uses multiple EMR platforms to gather patient information.
- Responds to emergencies as they occur in a timely fashion with clear delegation to clinic staff.
- Performs nursing assessment, implements a plan of care, and arranges for follow up care for primary care patients.
- Provides patient education on a variety of topics (e.g. preventative care, such as PrEP, doxyPEP, HIV, STI’s, harm reduction, colon cancer screening, diabetes, hypertension, asthma, nutrition, and other specific diagnostic procedures).
- Provides patient-centered and team-based medical management according to defined algorithms for patients with chronic diseases.
- Participates in the Enhanced Care Management (ECM) program to document medical case management delivered for reimbursement. Participates in care coordination meetings for ECM-enrolled patients.
- Provides wound care service under supervision of providers and clinical protocols on site.
- Participates in daily Quality Improvement activities.
- Performs initial and continued assessment of client health status, immunizations, and TB screening based on established protocols.
- Administers adult immunizations, medications, and phlebotomy consistent with current guidelines and established protocols.
- Provides culturally and racially competent client care in a manner that is both non-discriminatory and nonjudgmental.
- Communicates effectively with client/patient, clinic staff, the public at large and interdepartmental staff in a nondiscriminatory manner.
- Works with other members of the health care team to provide comprehensive primary care services.
- Participates in pre-clinic daily huddle, identifying high-needs patients for the clinic session.
- Covers clinical job duties of non-licensed staff when required by clinic management.
- Proficiently triages patients based on acuity, complexity, and immediate needs to appropriate Primary Care Provider.
- Links patient to appropriate in-house and outside medical, behavioral, and social services.
- Supervises vaccine management program in conjunction with Department of Public Health.
- Maintains accurate log of vaccines and therapeutic injection inventory, expiration dates and orders vaccines as needed.
- Conducts and or supervises administrative aspects of care coordination (eg. prior authorizations, specialty referrals, post-hospitalization discharge).
- Conducts and/or supervises clinic medication inventory and crash cart inventory.
- Evaluates and executes routine refills in a timely manner as well as prepares and submits prior authorizations according to established workflows.
- Coordinates treatment plans/procedures for routine services such as pre-exposure prophylaxis (PrEP), drug detox and medication assisted treatment of opioid use disorder, STI treatment, HIV treatment, and hepatitis C treatment.
- Coordinates follow-up care in accordance with treatment plan outlined by Primary Care Provider and interdisciplinary team.
- Evaluates patients for medication adherence by Primary Care Provider’s treatment plan. Conducts accurate and thorough medication reconciliation and medication management (such as dispensing medications weekly or DOT) as ordered by PCP.
- Provides N95 FIT testing for staff as needed.
- Serves as an integral member of the clinical and interdisciplinary team to provide wrap-around services/ care for clients in a team-based care model.
- Other job-related duties as assigned.
Physical Demands:
- Employees may need to occasionally lift up to 50 pounds unassisted.
- Requires clear close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus.
- Employees may be occasionally exposed to toxic or caustic chemicals.
Work Environment:
- Employees have direct contact with clients/patients while administering care.
- This position may require traveling to other office locations or off-site meetings as required by the job.
Minimum Qualifications:
- Associate or bachelor’s degree in nursing.
- Current license as Registered Nurse through the California Board of Registered Nursing.
- Current Basic Life Support Certification.
- Two years of experience as a Registered Nurse
- Effective written and verbal communication skills.
- Must have a philosophy that is consistent with the Mission, Vision, and Core Values of the organization.
- Ability to organize, plan, and effectively work on an interdisciplinary team.
- Must obtain TB clearance and complete background check upon hire.
- Up-to-date vaccinations including 2024 COVID-19 and seasonal flu
Preferred Qualifications:
- Experience/proficiency working with Epic EMR.
- Experience working with people of diverse backgrounds (gender identity, sexual orientation, race, ethnicity, immigration status).
- Experience working with clients affected by significant challenges with behavioral and/or mental health.
- Experience applying harm reduction principles in clinical care.
- Experience working with highly traumatized populations, using trauma-informed principles of care.
- Fluency in English and an additional language (e.g., Spanish, Arabic, Thai, Vietnamese, Tagalog, or Chinese).
- Experience working in an FQHC or other type of Community Health Center.
